Incident Form

This form is for all Open Homes Nottingham Accidents, Incidents, and Near Misses. That includes:

• All work-related injuries, diseases, ill health and near misses;
• All Nottingham Nightstop related major and minor accidents, incidents and near misses;
• All Supported Lodgings related major and minor accidents, incidents and near misses.

This form is to be completed immediately after any incident has occurred (or as soon as is reasonably practicable).

Completed forms will be reviewed by the Safeguarding Lead and/or the Health & Safety Lead as appropriate, who will advise if further details are required
